From the day of admission, a highly integrated support system provides for all physical and emotional needs of the patient. An interdisciplinary Care Plan Team, including Physical Therapists, Speech Therapists, Occupational Therapists, Nurses, Dieticians, Social Service Workers, Case Managers, and Activities Personnel work together to develop a plan of care and discharge. The patient's medical needs, physical abilities, emotional state of mind, family support and dietary needs are all taken into consideration to tailor a rehabilitation program specially for the needs of the individual patient. All those involved, including the family, work toward common goals.
